Job Description:  Residential Case Manager

Sign on bonus $2,000 for Full Time (30-40 hours). The bonus will be paid out in two increments!

Education/Experience/Training:   BA or BS in Human Services preferred. (High School diploma required) and a minimum of 2 years experience and/or training in working with individuals with physical, intellectual and developmental disabilities. 

General Description of Position Responsibilities:  The Case Manager works with individuals with physical, intellectual, developmental, and other disabilities in a supervised residential setting.  Under the direction of the Program Manager, the Case Manager is responsible for facilitating smooth operation of the program and ensuring the health and safety of the residents.  Also responsible for assisting with the development and implementation of  Individual Service Plan (ISP) objectives; maintaining up to date documentation of monthly progress notes, ensuring that all monthly tasks are performed, including Fire Drills, monthly rents and banking records. Also instructs, supervises, and assists individuals in A.D.L. skill acquisition; provides recreational and community integration opportunities. The Case Manager must promote the growth, independence and dignity of the individuals served and promote the individuals’ right to exercise choice and make meaningful decisions in their lives. The Case Manager’s duties will include direct care services to our individuals.

Requirements:

  • 1st Aid, CPR AND MAP certification required
  • Valid driver license required
  • Acceptable CORI, Abuse Registry, Driving Record, SS & OIG with Fingerprinting background check will be required.
  • Reliability and trustworthiness a must
  • Ability to physically assist/lift/transfer individuals safely
  • Effective written, verbal and interpersonal communication skills required
  • Ability to work as part of a team required
  • Computer proficiency using Microsoft Office and Database Software as well as internet skills
  • 1 year of caregiving experience required. .

Schedule:  Must be available to work evenings and weekends.  You are required to be on-call as needed.
